How do I get my child evaluation for services?
Your child can be evaluated for ABA services by a qualified prescribing professional such as physician, physician assistant, certified registered nurse practitioner, psychology or psychiatrist or another license individual such as a social worker, LPC or LMFT. If you feel your child could benefit from having in-home ABA support, talk to your prescribing professional about completing the written order for assessment.
How do I start services?
In order to receive ABA services, your child’s prescribing professional (physician, physician assistant, certified registered nurse practitioner, psychology or psychiatrist or another license individual such as a social worker, LPC or LMFT) will need to provide you with a written order (much like a prescription) detailing the type and amount of service needed.

This written order will need to be presented prior to beginning the assessment process to get authorization for a functional assessment (FBA). This FBA will provide us with valuable information needed in regard to your child’s needs, strengths, as well as the support needed to request and authorization for ongoing services. The information gathered will further help in the development of a treatment plan.

Evolutions ABA works with any child between the ages of 18 months and 21 years of age that has a medical necessity for ABA servcies due to various intellectual, developmental, and behavioral disabilities.
